Illinois is rewriting the playbook on what state leadership can do for its residents, and Gov. JB Pritzker is leading the charge with a bold, unprecedented move: wiping out medical debt for hundreds of thousands of people. Through careful negotiation and a partnership with the nonprofit Undue Medical Debt, the state has eliminated over $1 billion in medical debt for more than 500,000 Illinoisans, a level of direct financial relief rarely seen at the state level.

Residents don’t even need to apply—the program automatically identifies qualifying households and cancels their debts, giving families breathing room without the hassle of paperwork.

Eligibility is targeted but broad enough to make a real difference. Residents with household incomes up to 400% of the federal poverty line or debts amounting to 5% or more of their income qualify. The state buys these debts at pennies on the dollar and forgives them entirely, turning a modest investment into an outsized impact: for every $1 the state spends, more than $100 in debt disappears. The average relief per person comes to roughly $1,200, enough to relieve substantial financial pressure for families juggling medical bills, housing costs, and daily expenses.



This is more than just numbers on a balance sheet. Medical debt is one of the leading causes of financial stress and bankruptcy in the United States, and by eliminating it, Illinois is freeing people from a burden that can dictate choices about healthcare, education, and family stability. Families who once had to delay care or make impossible financial trade-offs now have the freedom to plan for the future, pursue new opportunities, and engage more fully with their communities.


The program is already showing results far beyond expectations. In just over a year since its launch, it has erased more than $1 billion in debt, a milestone that underscores the power of proactive, creative governance. With additional funds allocated for the current fiscal year and proposals to expand the initiative in the future, the state is setting a national example for what targeted, pragmatic policy can achieve.

In an era where financial pressures loom large over everyday Americans, Pritzker’s medical debt relief program is a rare, tangible win—a reminder that government, when bold and precise, can deliver real-life relief in a way that touches people’s wallets, hearts, and peace of mind.
